Pages that contain a cryptotoken are not inhibiting caching.

This can cause a stale token to be submitted if the page is refreshed, e.g. after an AJAX request consumes the original token.

Teach LoginManager to inhibit caching when a cryptotoken is present in a response.

In trunk, please merge to release branches.
