I registered a user "JohnSmith" in my dev TWiki. At the end I got this message:
Thank you for registering
That is, my own user name was shown, not "JohnSmith". Registration proceeded properly, e.g. user account has been created for "JohnSmith".
Using Apache login; all scripts are under authentication.

I happen to have the same configuration as you (as far as the available information goes) so I tried this (I was logged on as me)
I get
- Your personal TWiki topic JohnSmith has been created
You are going to have to be more explicit about how to reproduce this (configuration etc).

I reproduced it, and fixed it in 4.2 -

The 'thanks' message.tmpl used %WIKIUSERNAME% rather than the actual user topic created. So if you were already logged in, and registered a new user,
and where not using sessions support (Peter obviously does not 'need' sessions, as all scripts use apacheauth..) This would happen.
