This was the first plugin manually installed on my version of TWiki. I followed all the instructions, and when I did the reccommended test (added an IP to the BANLIST), it did not behave as documented:
1) Looking at a TWiki topic (with the view script) had the expected 1 minute wait, then the expected message - no problem here.
2) BUT I did not get the "500 Internal Server Error" for other scripts (login.pl, edit.pl) - they all had the same result as 1): a 1 min wait and message.
3) It let me go to configure.pl, and see all of the TWiki's settings, including the {AntiSpam}{EmailPadding}.

Where does it say that
BlacklistPlugin should give a 500 message?
We actually have a bug in 4.1.0 so
BlacklistPlugin fails to give a proper message. It will be fixed in 4.1.1. And it works fine in 4.0.5.
The plugin however does work except for the message for the bad guys.
If you can visit configure it has nothing to do with
BlacklistPlugin.
Then you have not followed the installation documents and protected configure properly in the apache configuration and this is a serious security issue for you.
